WASHINGTON — A bombshell report commissioned by the U.S. Department of Health and Human Services alleges that hospitals and gender clinics created profitable, long-term revenue streams by placing vulnerable children on puberty blockers, cross-sex hormones and surgical pathways while potentially using misleading insurance codes to secure payment.

The 64-page report, titled “Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of ‘Gender Medicine,’” was released Thursday. It examines hospital finances, insurance billing practices, the influence of medical organizations and the role of the Biden administration in expanding access to gender-related medical procedures for minors.

This is huge — and every parent in America should pay attention.

According to the report, hospitals and clinics submitted nearly $120 million in charges for gender-related treatments involving minors between 2019 and 2023. The underlying watchdog data cited by HHS identified more than 5,500 surgical procedures and over 8,500 minors who received hormones or puberty blockers.

Mount Sinai Medical Center in New York reportedly submitted more than $8.2 million in charges during that period, while Boston Children’s Hospital submitted approximately $6.5 million.

Those figures represent submitted charges, not necessarily the amount ultimately paid to the providers. However, the report argues that the financial incentive did not end with a single prescription or procedure.

Children who began these medical pathways could become long-term patients requiring repeated appointments, laboratory testing, medication monitoring, hormone prescriptions and possible surgeries extending into adulthood. HHS described that continuing treatment as a potentially valuable and dependable revenue stream for hospital systems.

In plain English: These children were not simply patients. They were potentially customers for life.

Report raises potential insurance fraud concerns

One of the most disturbing sections of the report concerns allegations that some medical providers may have used misleading diagnostic codes to obtain insurance coverage for procedures that otherwise might not have been covered.

HHS examined claims involving broad diagnoses such as “endocrine disorder, unspecified” and “precocious puberty.” The report alleges that such codes may have been used in some cases instead of diagnoses explicitly related to gender dysphoria.

Puberty blockers can be prescribed to children experiencing genuine precocious puberty, a medical condition in which puberty begins abnormally early. The HHS report raises questions about cases involving teenagers between 13 and 17 years old who received blockers accompanied by a precocious-puberty code.

According to HHS, using an unrelated or false diagnosis to obtain reimbursement could expose providers to civil or criminal liability under federal and state health care fraud laws.

The report does not establish that every provider or every claim it identified was fraudulent. It calls for further examination of suspicious billing patterns and recommends that state Medicaid programs conduct periodic claims reviews. It also urges referrals to Medicaid fraud-control units, state attorneys general or federal law enforcement when evidence of improper billing is discovered.

That distinction matters. An allegation must be investigated and proven. But the evidence presented is serious enough that government officials cannot simply look away.

Hospitals allegedly followed ideology and money

The report describes what it calls a dangerous combination of political ideology and financial incentives.

Medical organizations promoted gender-related treatments as necessary care while hospitals stood to receive revenue from consultations, prescription drugs, laboratory work, surgeries and lifelong follow-up treatment. Meanwhile, parents were often warned that refusing to affirm a child’s new identity could place that child at risk of suicide.

That framing left frightened families believing they faced only two choices: approve irreversible treatments or lose their child.

But children cannot legally purchase alcohol, sign most contracts or make countless other decisions reserved for adults. Yet medical professionals were willing to place some of them on pathways carrying possible lifelong consequences, including infertility, sexual dysfunction, altered bone development and permanent physical changes.

Children experiencing distress deserve compassion, careful counseling and truthful medical information. They should never become experiments for activists or revenue sources for institutions.

Biden administration helped expand the system

The report also accuses the Biden administration of applying federal pressure to expand access to these procedures.

According to HHS, the administration used federal agencies, civil-rights guidance and legal action to challenge restrictions on gender-related treatments. The report argues that those policies encouraged hospitals to continue or expand pediatric gender programs even as other countries began reconsidering medical interventions for minors.

The federal government was not merely standing on the sidelines. It was helping create the environment in which this system grew.

The current HHS report follows an earlier federal review that found weak evidence supporting the benefits of pediatric medical transition while identifying potentially serious and irreversible harms. That earlier review was released in May 2025 and later underwent peer review.

Accountability must follow the evidence

The newest report includes accounts from former patients and parents who say they were rushed through a system that affirmed a child’s stated identity without adequately examining trauma, mental-health conditions, family circumstances or other possible causes of distress.

Some now live with permanent physical complications. Others say the doctors and clinics that once encouraged their transitions stopped responding when they later sought help to detransition.

That is not compassionate health care.

If hospitals knowingly used false diagnoses to bill Medicaid or private insurers, there must be prosecutions. If medical professionals misled parents about risks and outcomes, there must be consequences. If taxpayer money financed procedures through fraudulent claims, that money should be recovered.

Doctors take an oath to do no harm. Hospitals are supposed to heal the sick, not manufacture lifelong patients.

The people named or implicated in this report are entitled to respond, and every allegation must be examined through a lawful investigation. But protecting children must come before protecting institutions, professional associations or political agendas.

For years, parents who questioned these procedures were labeled hateful, uninformed or dangerous. Whistleblowers were attacked, and people who demanded evidence were told the science was settled.

Now the government’s own report says ideology, financial incentives and potentially fraudulent billing practices may have helped drive the rapid medicalization of America’s children.

This was never merely about acceptance. It was about who benefited, who paid and who will carry the damage for the rest of their lives.

America needs the full truth. The investigations must begin, the records must be examined and those who exploited vulnerable children must be held accountable.
