Coffee Linked to Lower Liver Disease Risk in Scientific Review

Regular coffee consumption may help protect the liver and reduce the risk of several liver diseases, according to a scientific review published in Biochemical Pharmacology.

The review analyzed decades of epidemiological, experimental and clinical research and found that coffee intake is consistently associated with better liver health outcomes. Researchers reported links between coffee consumption and reduced risk of liver fibrosis, cirrhosis and hepatocellular carcinoma, the most common form of liver cancer.

The authors said the benefits appear to be dose-dependent, with protective associations observed at moderate intake levels.

Coffee contains numerous bioactive compounds, including chlorogenic acid and polyphenols, that may influence liver function. According to the review, these compounds appear to affect molecular pathways involved in oxidative stress, inflammation, fibrosis, metabolism and gut microbiome balance.

Those mechanisms may help explain why coffee drinkers often show better liver-related markers in population studies.

Oxidative stress and chronic inflammation are major drivers of liver injury. When those processes persist, they can contribute to scarring, fibrosis and eventual cirrhosis. The review found that compounds in coffee may help reduce those damaging processes through antioxidant and anti-inflammatory activity.

The review also identified antifibrotic pathways as a key area of interest. Fibrosis occurs when repeated liver injury causes scar tissue to build up. Over time, fibrosis can progress to cirrhosis, a serious condition that can lead to liver failure or cancer.

Researchers said coffee’s bioactive compounds may help slow or limit some of the processes involved in scar tissue formation.

Population-based studies have found that people who drink more than two cups of coffee per day tend to have lower serum liver enzyme levels. Liver enzymes such as AST, ALT and GGT are commonly used as markers of liver stress or injury.

Earlier research has found that people who drink three cups of coffee daily are less likely to have abnormal liver enzyme levels. Regular coffee drinkers have also shown lower levels of AST, GGT and ALT, markers associated with alcoholic liver disease severity.

The review also found associations between coffee consumption and improved outcomes in people with chronic hepatitis C. Patients with hepatitis C who consumed coffee daily had lower rates of progression to fibrosis and cirrhosis. Drinking two cups per day was linked to reduced risk of developing hepatocellular carcinoma in that group.

Coffee intake was also associated with lower risk of metabolic dysfunction-associated steatotic liver disease, or MASLD, compared with nondrinkers. MASLD, formerly commonly referred to as nonalcoholic fatty liver disease, is linked to obesity, insulin resistance and metabolic dysfunction.

The review suggests coffee may be a practical dietary factor for people at risk of liver disease, especially when combined with other lifestyle changes such as maintaining a healthy weight, limiting alcohol intake, exercising and improving diet quality.

Researchers cautioned, however, that coffee is not a treatment or cure for liver disease.

The review noted that caffeine tolerance varies from person to person. General safety guidance often places moderate intake at up to about 400 milligrams of caffeine per day, roughly three to four cups of coffee. Some individuals may experience side effects at lower amounts, including anxiety, heart palpitations, sleep disruption, stomach upset or reflux.

The authors also noted that very high coffee intake may carry potential drawbacks. Consuming more than five cups daily may increase LDL cholesterol in some people and could contribute to dyslipidemia. Evidence on the relationship between coffee and LDL cholesterol remains mixed, and preparation methods may matter.

Unfiltered coffee, such as French press or boiled coffee, can contain compounds that raise cholesterol more than filtered coffee. People with cholesterol concerns may want to choose filtered coffee and discuss intake with a healthcare provider.

The review concluded that coffee’s liver-related effects deserve further clinical investigation. Researchers said identifying the molecular pathways affected by coffee could help guide future randomized trials and clarify whether specific compounds, doses or preparation methods provide the greatest benefit.

For now, the evidence suggests that regular, moderate coffee consumption is associated with better liver health and lower risk of several liver diseases.

The findings add coffee to a growing list of everyday dietary factors that may influence long-term liver outcomes, while reinforcing that its benefits are most likely to matter as part of a broader healthy lifestyle.
