The NHS is bracing for a report on the biggest maternity scandal in its history, which is due to be published Wednesday.

An inquiry led by senior midwife Donna Ockenden looked at 2,500 cases involving babies and mothers who died or were injured while under the care of Nottingham University Hospitals NHS Trust. And her findings are expected to make for harrowing reading.
