Trooper's Report:

On 08/26/2024 at approximately 15:20 I Trooper Jeremy A. Elliott conducted a traffic stop on a white Chevrolet van at the
intersection of State Highway 109 and State Highway 309. The reason for the stop was the male driver (David Lewis
09/09/1962) was suspended.

When I activated my emergency lights, Mr. Lewis put his hand out of the driver-side window and (flipped me off). Mr. Lewis
proceeded to accelerate quickly and travel southbound on Hwy 109. While traveling southbound Mr. Lewis tossed multiple bags
out of the vehicle, crossed the center line, traveled at speeds of 100 miles per hour, and failed to signal multiple times.

While in pursuit of Mr. Lewis, I attempted to TVI (Tactical Vehicle Intervention) Mr. Lewis. When I did so, Mr. Lewis slammed on
his brakes which caused me to pass Mr. Lewis. He proceeded to speed up and pass me. I maneuvered and attempted to TVI
Mr. Lewis. I made contact with his rear driver-side bumper, but he was able to correct the TVI and continued southbound on
109.

Mr. Lewis attempted to turn westbound on Cox Valley Road from Hwy 109. When he turned he lost control of his vehicle and
ran off the road into the southern ditch of Cox Valley Rd. When he collided with the ditch, his vehicle rolled onto its passenger
side and stopped. I turned onto the dirt road and attempted to stop. I was unable to come to a complete stop before running
into the back of Mr. Lewis.

I conducted a felony takedown and placed Mr. Lewis under arrest without further incident. I searched the vehicle and didn't
locate anything inside.

Mr. Lewis was transported to the Logan County Detention Center and charged with the following.

5-54-125(d)(2) 2023: FLEEING - VEHICLE OR CONVEYANCE - EXCEEDING THE SPEED LIMIT
27-50-302(a)(2): RECKLESS DRIVING
8-6-406 1ST: LITTERING - 1ST OFFENSE
27-51-403: SIGNALS FOR TURNING, STOPPING, OR DECREASING SPEED REQUIRED
