Prostaglandins and Prostacyclins are responsible for the perception of pain, causing inflammation, and inducing fever.

The effects of diclofenac involve various organ systems.
In the cardiovascular system, prostaglandins exert cardioprotective effects by preventing thrombus formation, reducing atherosclerosis, and lowering blood pressure.
Inhibition of prostaglandin synthesis by diclofenac will cut off these cardioprotective effects and increase the risk of myocardial infarction, heart failure, and stroke.
In the gastrointestinal tract, reduced prostaglandin synthesis may lead to mucosal damage and gastrointestinal bleeding.
In the kidneys, diclofenac maintains adequate renal perfusion.
Reduced prostaglandin synthesis in the kidneys may reduce renal blood flow and glomerular filtration rate, which can manifest as acute kidney injury.
The risk is further increased in patients with previous kidney damage.
Other adverse effects include drug-induced liver damage, neutropenia and aplastic anemia with long-term use, and dermatological effects such as skin irritation with topical application of diclofenac.
