Paronychia is defined as inflammation of periungual tissues.
It results from the disruption of the protective barrier between the nail plate and the surrounding skin, allowing pathogens to invade.
Paronychia can be divided into acute and chronic.
Acute Paronychia is typically caused by bacterial infection, most commonly Staphylococcus aureus and sometimes Streptococcus species or Pseudomonas species.
It is characterized by painful, red, swollen area around the nail, often with accumulation of pus.
Chronic Paronychia usually occurs due to repeated exposure to moisture and irritants, and it often involves a mixed infection.
Unlike acute paronychia, chronic paronychia typically lasts more than 6 weeks.
The treatment of acute paronychia involves the following steps.
Warm Soaks.
Soak the affected finger in warm water 3 to 4 times daily to reduce inflammation and promote drainage.
Topical Antibiotics.
Mupirocin ointment or fusidic acid can be used for mild cases.
Oral Antibiotics.
Only if there is extensive cellulitis or systemic symptoms.
Examples include flucloxacillin, second-generation cephalosporins, or penicillins.
Incision and Drainage.
If there is a localized abscess or pus collection.
And analgesics for pain relief.
Topical Steroids: Low-to-medium potency to reduce inflammation.
