A report published by The Times states that Iran’s recently appointed Supreme Leader, Mojtaba Khamenei, is incapacitated and undergoing medical treatment in the city of Qom, according to information derived from a diplomatic memo shared among regional allies.
The memo, described as drawing on American and Israeli intelligence, indicates Khamenei is suffering from a “severe condition” and is “unable to be involved in any decision making by the regime.” The report characterizes the situation as limiting his ability to exercise authority following his installation as Iran’s top leader.
Qom, located approximately 85 miles south of Tehran, holds deep religious and historical importance within Shiite Islam. The city is home to major religious institutions and has longstanding significance dating back to the 1979 Islamic Revolution led by Ayatollah Ruhollah Khomeini, as detailed in historical accounts such as those documented by Britannica.
The development comes amid ongoing military activity in the region. According to reporting on recent strikes in Iran, including coverage from Al Jazeera, Qom was among locations targeted in air operations, with local officials stating that at least one residential building was struck. The memo cited in the report did not identify the precise location where Khamenei is receiving treatment within the city.
Mojtaba Khamenei, 56, assumed leadership on March 9 following the death of his father, Ayatollah Ali Khamenei, who was killed during the opening phase of Operation Epic Fury on February 28. Since his appointment, there have been ongoing questions surrounding his condition, as he has not appeared publicly or delivered verified statements directly.
Communications attributed to him have been limited to written messages, often delivered by intermediaries. He did not attend the formal ceremony marking his elevation to Supreme Leader, during which officials instead relied on visual representations in his absence.
The diplomatic memo also referenced preparations for a large mausoleum in Qom intended for Ali Khamenei and potentially other family members killed in the strike. Earlier reporting from Iranian sources had indicated plans for burial in Mashhad, along with ceremonies in Tehran.
Statements attributed to Mojtaba Khamenei in recent weeks have focused on continued support for Iran’s regional posture. According to reporting from the New York Post, he previously vowed to continue backing allied militant groups across the Middle East.
The reported condition of Iran’s Supreme Leader adds uncertainty to the country’s leadership structure at a time of heightened conflict and international pressure.
